    摘要:

    为对比分析菱形与非平行空心加压螺钉治疗三种类型(PauwelsⅠ、Ⅱ、Ⅲ)股骨颈骨折在模拟生物载荷下的力学差异。通过使用计算机三维处理软件(Minics、Solideworks、Geomagic)分析临床提供的CT数据进行建模,包括三种类型(PauwelsⅠ、Ⅱ、Ⅲ)及两种不同排列方式的空心加压螺钉模型;导入 Ansys进行网格划分、材料赋值,模拟生理载荷,观察最大形变和内固定的等效应力和剪切应力的方法。对于菱形和非平行空心钉模型,在PauwelsⅠ型(20度)骨折中,最大形变0.5602mm、0.5610mm,最大等效应力23.68Mpa、24.49Mpa,最大剪切应力3.715Mpa、4.160Mpa;在PauwelsⅡ型(40度)骨折中,最大形变0.5620mm、0.5653mm,最大等效应力24.64Mpa、25.32Mpa,最大剪切应力4.540Mpa、5.549Mpa;在PauwelsⅢ型(60度)骨折中,最大形变0.5708mm、0.5723mm,最大等效应力25.91Mpa、26.08Mpa,最大剪切应力6.668Mpa、7.089Mpa。可见在同等受力条件下,菱形空心钉模型的形变更小,内固定所承担的等效应力、剪切力更小,应力分布更为平均,更具有力学稳定性。

    Abstract:

    In order to compare the biomechanical properties of four screws internal fixation for femoral neck fracture of Pauwels typeⅠ、Ⅱ、Ⅲ:diamond and nonparallel screws,The clinical CT images were used to establish models of three kinds of different Pauwels types and two kinds of configuration models through three-dimensional virtual software(Minics、Solideworks、Geomagic).Models were imported into software Ansys to assume material properties and load pressure.The maximum displacement,maximum stress and shear stress were observed. The biomechanical properties of diamond and nonparallel fixation with cannulated screws were different。Among PauwelsⅠfracture(20 degrees),the max displacement were 0.5602mm、0.5610mm,the max equivalent stresses were 23.68Mpa、24.49Mpa,the max shear stresses were 3.715Mpa、4.160Mpa.Among PauwelsⅡfracture(40 degrees),the max displacement were 0.5620mm、0.5653mm,the max equivalent stresses were 24.64Mpa、25.32Mpa,the max shear stresses were4.540Mpa、5.549Mpa.Among PauwelsⅢfracture(60 degrees),the max displacement were 0.5708mm、0.5723mm,the max equivalent stresses were 25.91Mpa、26.08Mpa,the max shear stresses were 6.668Mpa、7.089Mpa. It is concluded The diamond configuration models has better stress dispersion effects , less displacement and shear stress conditions,better biomechanical stability under the same stress.
